[en] The efficacy of radiation therapy is to eradicate the tumor while preserving the integrity of normal tissues. Technological advances have allowed to develop techniques capable of modulating doses delivered to the target volume, providing more effective treatments. However, the operational complexity of these techniques makes the benefits offered are directly proportional to the chances of occurrences of serious errors. The objective of this work is to analyze the steps performed by the technician/technologist in Intensity Modulated Radiotherapy (IMRT), to detect possible errors in order to determine ways to mitigate them. After literature regarding errors in the radiation therapy, a prospective analysis was performed in the first half of 2012 in a radiation clinic located in the city of Rio de Janeiro, in which 11 technicians/technologists contributed to the survey data analysis. The method of risk analysis Failure Mode and Effects Analysis was used for prospective analysis of accidents/incidents, with respect to a qualitative assessment . The method allowed mapping 16 steps performed by technicians/technologists in the treatments with IMRT, identifying possible failures and their causes allowing to find ways to avoid possible errors. This analysis helped to confirm that the qualification and continuing education of technicians/technologists, allied to implement quality assurance programs and a computerized management can make a tool capable of IMRT to achieve the greatest challenge of radiotherapy.
