AbstractAbstract
[en] The paper discusses selective bronchial angiograms in 21 breast cancer patients, aged 35 to 54 years, undergoing surgery and irradiated on a ''Rocus'' gamma teletherapeutic unit with three direct and two tangential fields, at total focal doses of 4500-5000 rad delivered over a 7- to 9-week period. Selective bronchial arteriography on the respective side was performed after Zebdlinger at various time intervals following completion of the radiotherapy course, namely within 2-3 months in 10 patients and within about one year in 11 patients. At early stages (2-3 months after radiotherapy), angiograms showed enlargement of the main trunk of the bronchial artery and its branchings, as well as development of an abundant interstitial vascular web in the hilar area in 9 patients and in the subclavicular area in 4 patients. In two of these cases direct anastomosing was observed between branches of the bronchial artery and those of the pulmonary artery, as identified by their typical course. Three patients displayed focal clusterings of pathological vessels suspect for metastases to pulmonary hiluses or parenchyma. At later stages (1 year) after radiotherapy, angiograms typically showed segmentary lumen constrictions of the bronchial artery and its branches, a zigzag course, relative depletion of the angiographic pattern, and delayed contrast medium passage. In 2 patients from this group, contrast filling of intercostal arteries allowed visualization of recurrences in soft tissues. Bronchial angiography should be a useful means for complex study of the characteristics and evolution of radiation pneumonitis. (author)
